If you or a family member are interested in joining a PACE Program, the following eligibility criteria must be met.

• Must be 55 and older

• Must require nursing home level of care, including more than 120 days of community based long-term care services. You also must be able to live safely in your own home.

• Must be covered by Medicaid or Dually-Eligible. If you’re not eligible for Medicaid and would still like to enroll, you must be capable of private-paying for Medicaid services provided by your PACE IDT.

• Must live in eligible service area. Please check our members page to see if there’s a PACE Center near you.
